Manager, Care Management

You help breathe to life the passion in caregiving. As a Manager of Care Management you are engaged in our desire to provide the best managerial support for your team. You are also curious about new ways to provide supports to the elderly and people with disabilities. You want to be on the forefront of change by building into care managers who work in the homes of our families. You want to manage a team of clinical and non-clinical care managers. We need someone that has a bachelor’s degree or equivalent, preferably in a health care or social services discipline. You will have 7+ years of case management experience in healthcare and social services, preferably with experience in eldercare, and/or people with disabilities or other complex medical needs. You will have at least 3+ years of experience managing, coaching or developing staff; 2+ years of program or clinical operations management. An active RN license is strongly preferred.
